Notice to Potential Ineligible Voters

Notice is hereby given to the following persons at the last known address of:

Pursuant to Section 98.075, Florida statutes, you are hereby notified that you are potentially ineligible to be registered to vote.  Failure to respond within thirty (30) days after this notice is published may result in a determination of ineligibility by the Nassau County Supervisor of Elections and removal of your name as an active voter from the statewide voter registration system.  Please contact the Supervisor no later than thirty (30) days after the date of this published notice to receive information regarding the basis for the potential ineligibility and the procedure to resolve this matter.  If further assistance is needed, contact our office at 904.491.7500.

If you deny the accuracy of the information underlying the potential ineligibility, you have a right to request a hearing for the purpose of determining eligibility.

If you attempt to vote at an early voting site or your normal election day polling place, you will be required to vote a provisional ballot.  If you vote by mail, your ballot will be treated as a provisional ballot.  In either case, your ballot may not be counted until a final determination of eligibility is made.  If you wish for your ballot to be counted, you must contact the supervisor of elections office within 2 days after the election and present evidence that you are eligible to vote.
